Het is weer Halloween en Michael Myers keert terug naar zijn geboortedorp, Haddonfield, om wat onafgemaakte ''familiezaken'' op te lossen. Zijn zus Laurie Strode is in het ziekenhuis beland, na haar laatste ontmoeting met Michael. Ze denkt dat ze hem heeft vermoord, maar de nu gehavende Michael Myers leeft nog en zoekt zijn zus op in het ziekenhuis. Daar laat Michael een spoor van dood en verderf achter, maar Laurie weet te ontsnappen. Terwijl zijn jeugdherinneringen hem achtervolgen en hij weer in contact komt met zijn moeder lijkt niets Michael te kunnen stoppen om zijn gestoorde verleden achter zich te laten...
